POPPA, Inc is an independent 501(c)3 non-profit that offers a statewide spay/neuter referral and assistance program for the state of Oregon. We are not affiliated with nor do we receive funding from any Humane Society or County Animal Control. We rely on grants, private donations and plant sales at Recycled Gardens.

2) Please provide the following info, as it applies:
2A) For cats please provide your pet's sex, approximate age, and name. For females please also specify if they are feral, in heat, pregnant or if they have had any litters and the date of the last litter.
2B) For dogs please provide breed (or mixture of), sex, approximate age, weight, and name. For females please also specify if they are in heat, pregnant or if they have had any litters and the date of the last litter. Please also specify where and how this dog was acquired (please be specific).
2C) For other pets please provide species, sex, approximate age and name.
3) How will you transport your pets to the clinic? (i.e. car, bus, walk, cab, friend)
3B) In the event that we need to send you out of your area to provide the requested level of assistance, please list 3 or more communities in addition to your own where you could take these animals for spay/neuter.
